As far for sheer expense, you will fine that national flag-carriers are perhaps the most expensive when it comes to fares. For example, first-class on an airline such as Alitalia would be quite costly.
As for being eco-friendly, I must admit that Virgin Atlantic is doing all that it can to take steps to the future, so much so that Sir Richard Branson stated that in the future he wishes to eliminate all 4 engined aircraft (which are more powerfull and do not have ETOPS constraints) or more fuel-efficent 2 engines aircraft. Also, he propsed that he will be considering using taxing all of his aircraft directly to the runway using runway tugs (rather than having the aircraft use engine power to get to the runway) thus saving fuel and being much more eco-friendly.
Apart from that, in the past few years we have seen a new trend emerging in the market - all business class carriers such as L'avion, MaxJet, SilverJet e.t.c that serve the high-demand U.S. to Europe routes. The carriers offer all business class cabins, and tickets often start at above-business class (at regular airline) costs.
Also, as for very luxurious airlines -
"The Gulf Carriers" - most notably Emirates, and also Etihad, Qatar Airways, and Gulf Airways. Seeing that those carriers are subsidized by their individual home states, and futher that they don't have to worry about fuel costs, they maximize on luxary. Emirate's first class product is world renown.
